FORMS

Liability Form
If a class asks for liability forms to be filled out and turned in to the instructor - please click on the following link, print the form and fill out and take to the first class. All activity and dance classes require these forms and also a few other classes. The other classes will have a special note saying a form is required.

Minor Consent Form
Students ages 14-17 can take a variety of in-person adult class with the approval of the instructor (contact our office and we will contact the instructor) and the Minor Consent forms filled out and submitted at the same time of registration.

Swim Consent Forms       
These forms must be filled out, signed, and turned in on the first day of swim lessons.
Please fill out the forms prior to class to make first-day check-ins run smoothly.
